Excursion and Activity Consent Form
Use this service to:
- Let us know you consent to excursions with the Youth Participation Team
- Provide details the team will need to keep everyone safe while Young People take part in the activities
- Share information, such as contact detail so we know how to contact you
Your Young Person may participate in multiple excursions with the team. This form will cover any excursions for 12 months from the date it is submitted. You will be notified of any excursions by letter, email or text message and you will have a choice to opt out of that excursion by contacting the team.
If the young person is under 16 this form must be completed by parent/guardian. Please note that consent is required from a Young Person if they are aged 12 or over, with regards to their personal data. If your Young Person is aged between 12 and 16 please discuss and agree to recorded imagery consent with them before completing this form. If the young person is 16 or over the young person can complete this form by themselves.
The form will take around 5 minutes.
Stirling Council, Senior Phase and Youth Participation takes its responsibilities regarding the protection of personal privacy very seriously. As such, you can be rest assured that any personal data that you have shared will be held and processed in accordance with the requirements of the Data Protection Act 2018 together with UK GDPR. The service holds data about service users in the order to ensure that appropriate procedures can be enacted in the case of an emergency and to inform them and their parents/carers of service development.
Where you have submitted personal data, such as contact details, postal address or a telephone number, the data will not be disclosed to any third party without your consent.
